Thank you for your interest in MyChart at Cincinnati Children's Hospital Medical Center. MyChart is an easy-to-use, online tool that provides a mini-view of your/your child's Cincinnati Children's medical information.
BEFORE YOU GET STARTED:
The patient must be an established patient at Cincinnati Children's in order to request a MyChart account online.
Requests will not be reviewed and approved without a valid form of photo identification.
A driver's license, passport, school ID (patients 13-17 only), and state-issued identity card are all valid forms of photo identification. Theme-park passes and basic photos will not be accepted.
Appropriate proof of ongoing legal guardianship is required to obtain MyChart access to patients who are 18 and older with diminished capacity. Documentation can be submitted using our secure upload page.
The request form and photo ID will be reviewed within 7 business days of submission. If approved, an activation code will be sent via secure email. If there are questions regarding the information provided on the form, you will be contacted via phone or email. IMPORTANT: Online account requests cannot be expedited.
