MyChart Activation Code Request

Thank you for your interest in MyChart at Cincinnati Children's Hospital Medical Center. MyChart is an easy-to-use, online tool that provides a mini-view of your/your child's Cincinnati Children's medical information.

To sign up for MyChart access to your or your child(ren)'s record, please select the type of access you are requesting, then complete and submit the online form for approval. IMPORTANT: The patient must be an established patient at Cincinnati Children's in order to request a MyChart account online. Requests will not be reviewed and approved without a valid form of photo identification.

Once the request form and proof of photo ID have been received, reviewed and approved, we will send an activation code through secure email within 7 business days. We will contact you by phone or email if we have any questions regarding the information provided on the form.

Who May Request MyChart Access Online

Parents/Primary Caregivers of Patients 12 and Under: A parent/primary caregiver may be granted full proxy access to MyChart. Patients under 12 may not have their own MyChart account.

Parents/Primary Caregivers of Patients 17 and Under with Diminished Capacity: A parent/primary caregiver of a child 17 years old and younger who has diminished capacity may be granted full proxy access to MyChart. Diminished capacity must be verified by a physician. This is managed through our Health Information Management (HIM) Department at Cincinnati Children’s. Simply check the appropriate box on the form to indicate diminished capacity for our staff.

Patients 18 and older: MyChart proxy access expires when the patient turns 18. Patients can request MyChart access for themselves at any time - and patient access to MyChart never expires.

Parents with proof of ongoing legal guardianship may be granted MyChart proxy access, which does not expire.

A patient can also choose to sign the MyChart Adult Proxy Access Form to grant access to another person. Adult Proxy Access expires every 18 months and must be renewed in person by the patient.

Types of Access That Must Be Requested In Person

Patients 13-17: Patients who are 13 to 17 and are not of diminished capacity may sign the MyChart Assent form to grant full proxy access to a parent or primary caregiver. The patient may also have an account once the MyChart Assent form is signed. For patients age 13-17 who are not of diminished capacity, MyChart access must be requested and created in person. Please ask one of our front-desk staff, or stop by the Cincinnati Children’s Family Resource Center, during your next visit.

Can a foster parent access his/her foster child's records in MyChart?

No, unfortunately. MyChart access cannot be granted for children in temporary guardianship situations.
